Adult Medicaid Service CoordinationMedicaid Service Coordination (MSC) is an important service in the life of a person with developmental and/or intellectual disabilities. Our Service Coordinators are qualified to serve individuals of all ages, currently providing services to over 200 adults and children. Under MSC, parents and family members are recognized as valuable participants in an individual’s service coordination program. The focus of MSC is to identify needs; to assist people to achieve their unique goals; and, to obtain appropriate services and supports.

Our Qualified and Caring Medicaid Service Coordinators:

  • Meet with individuals and their families to assist them in locating and accessing community services such as Medicaid, Social Security, Family Reimbursement, etc.
  • Help individuals find services and programs based on their choices and needs
  • Use a “person-centered” approach in developing, implementing, and maintaining an
    Individualized Service Plan (ISP)
  • Promote the concepts of choice, individualized services and supports, and consumer satisfaction
  • Offer services to individuals who are also part of others’ and On Your Mark‘s Residential Program
